Participants in the program receive:

  • Ongoing educational mailings about the condition
  • Phone access to a nurse for specific questions and to discuss preventive and self-care strategies
  • Ongoing surveillance using our predictive modeling software

Members are automatically enrolled in the program based on claims information.

AsthmaCare

Asthma is a disease that makes it difficult to breathe. Without proper treatment, asthma interferes with a person’s ability to carry out normal activities and leads to increased use of medical services, such as office visits, outpatient services, emergency services and hospital admissions. DiabetesCare

Diabetes is a disease in which the body does not produce or properly use insulin, a hormone that is needed to convert sugar and starches into energy for daily activities. DepressionCare

Depression is a serious illness that affects a person’s mood and interferes with relationships, home-life and work. If left untreated, depression can make it hard to carry out self-management and preventive health practices, causing existing conditions to worsen.
