Welcome to the care model site. A care model is a comprehensive evidence-based approach to patient care delivery. It promotes right care by the right provider in the right care setting in a coordinated manner. Care models support the Triple Aim: health outcomes, affordability and consumer experience. 
Care models are not meant to substitute for clinical expertise in treating the specific needs of an individual patient nor are they meant to serve as a legalistic standard of care; they do serve as a resource on current best practices.

Care models at AnewCare are developed by a team of representative physicians with approval of the AnewCare board. They will be reviewed on a yearly basis for any changes in the evidence.
